Affidavit of service (complaint) - Form 6A

Affidavit/declaration to be sworn by person serving a complaint to verify service of that complaint.

Note: Informal service under Rule 6.11 is not a substitution for personal service of a complaint (required under Rule 6.02). Rule 6.11 simply allows for a document that has not been able to be served as required by the rules to be deemed served if the document has come to the notice of the applicable person. Emailing a complaint to a person to bring it to their attention instead of attempting to personally serve does not constitute proper service.
