Koori Court referral form
A Koori Court referral form must be filed with the relevant Magistrates’ Court before a Koori Court Officer can start work on case.
Application for appearance by audio visual link - Form 46
This application is to be used if an accused has been ordered to appear physically in the Court but an appearance via AVL is now sought. The application may be made at any time up to 14 days before the court hearing and must be served on the other party.

Application for physical appearance - Form 45
This application is to be used if an accused has been ordered to appear by AVL on the return date but a physical appearance is now required. This application may be made at any time up to three days before the court hearing (the Court may reduce this time further if there is sufficient reason) and must be served on the other party.
